What to Expect

You have already begun your journey by seeking out information pertaining to home based Speech and Language therapy. Below I have outlined the process that will bring you one step closer to improving your communication skills.

Before First Visit

Once you have submitted your contact information, I will reach out to you via phone call. We can discuss your situation to determine our next steps and schedule your first visit.

I will send you some intake forms to complete prior to first visit.

  • Child Intake Form / History

  • Acknowledgement That You Have Received Our HIPAA Privacy Notice

  • Authorization to Exchange, Obtain or Release Information

First Visit

During our fist visit, I will work to learn about your individual needs.  This will involve formal testing, observations including motor speech movements and/or language sampling through a spontaneous speech sampling.

  • Independent Contractor Agreement

Second Visit

During our second visit I will share a written report pertaining to the results of the evaluation.  We will discuss based on the results a treatment plan.  In addition, we will discuss when we will review/revise the treatment plan.

Parental Expectations

Speech language intervention has the most success when partnered with the family and client.  When both parties work together expectations for improvement occur faster.  For this reason, I am coming into your home to work directly with your loved one but also to provide you with techniques, strategies and idea's for practice beyond just the speech therapy session.
